(Optional) You may order via the DANTES Material Request Form (stock number 4301). Call DANTES at DSN 922-1082 or Commercial (850) 452-1082 to receive your copy of this form.
When ordering by any means, give the DANTES Stock Number for the item(s) ordered, quantity needed, and your DANTES ID Number.
Steps to Obtain a DANTES ID# for DoD Activities
Contact Comm (850) 452-1082 or DSN 922-1082, or e-mail dist@navy.mil.
A DANTES ID Application will be sent, (e-mail or fax), whichever is most convenient for you.
When you receive the application, fill out all blocks completely and return by fax (850) 452-1140, or e-mail dist@navy.mil.
Upon our receipt, a DANTES ID # will be assigned to your command. An official letter will be sent notifying you of the assigned DANTES ID Number.
Ordering Testing Materials for Civilians and Military Family Members
Testing of civilians or military family members is not permitted on any instruments ordered from DANTES. Only materials ordered directly from the suppliers and paid for by the individual completing the order form or purchase order can be used with civilians and military family members. DANTES will not pay for any materials ordered directly from the contractor. It is very important, therefore, to be sure you are ordering the materials from the correct source.
